SCHOOL YEAR FORMS
Forms must be submitted by the first day of school.
ALL STUDENTS (New and Returning)
NEW STUDENTS AND RETURNING STUDENTS
If you have already submitted any of the following forms, we do NOT need them unless there are updates:
- Bring in photo ID of your child: birth certificate or passport
- School Entrance Health Form (signed by a doctor)
- Emergency Medical Treatment Form
- EpiPen Authorization Form: Only if your child needs an EpiPen
- Benadryl Authorization Form: Only if your child needs Benadryl for allergies
- Emergency Rescue Inhaler Form: Only if your child needs a Rescue Inhaler
